OBJECTIVE: > No use of accessory muscle > Irregular breathing > Breath sounds: wheezes > Weak peripheral pulse

NURSING INTERVENTION Ineffective breathing pattern related to respiratory muscle fatigue as manifested by irregular breathing.

PLANNING LONG TERM: After 1 week of effective nursing intervention the patient will be able to: > Establish a normal respiratory pattern as evidenced by other sign and symptoms of hypoxia. SHORT TERM: After 3 days the patient will be able: > Demonstrate appropriate coping mechanisms. •

RATIONALE

DEPENDENT: Auscultate chest to evaluate presence of breath sounds.

> To identify the character of breath sound

Monitor rate and depth of respirations, type of breathing pattern.

INDEPENDENT: >Medicate with analgesics ordered by the physician.

EVALUATION GOAL MET:

>Inadequate oxygenation causes increase PR. >to identify the type of breathing pattern
