FIRE AND LIFE SAFETY ASESSMENT REPORT, PART 3 Post Construction Phase (FALAR, PART 3) A. OBJECTIVE To maintain a well secured and safe building facilities from unwanted fire and other emergency hazards. To monitor the standards set under the Fire Code of the Philippines, to measure the data’s in order to maintain a well secured and safe facility. B. DESCRIPTION OF BUILDING AND FACILITIES A factory type of building for manufacturing of Plastic Products which comprises of a one (1) storey building with two (2) partitions. The building A comprise of the istration division and the production with 1,170.00 SQM floor area, while building B is leased by ZPI PLASTIC CORPORATION with 810 SQM floor area. The facility is located at lot 15 block 20 phase IV inside Cavite Economic Zone under Philippine Economic Zone Authority in Rosario, Cavite which comprises of 3,241.00 square meters total area. C. STATEMENT ON WHO IS ENFORCING AUTHORITY FOR THE NATIONAL BUILDING CODE AND THE FIRE CODE The PEZA EHSD (Cavite) is enforcing the authority for the National Building Code and the Fire Code of the Philippines that the company complying to. As the requirement to this, the Company has its own Fire Insurance Policy as compliance which is under the Philippine Fire and Marine Insurance Corporation. D. STATEMENT OF TESTING AND MAINTENANCE STANDARDS USED The company used the NFPA 10 Standard for Portable Fire Extinguishers 2007 Edition guide to portable Fire extinguisher in maintenance and testing of the Fire extinguishers and monthly monitoring and checking of the exits and other form and means for Fire emergency usage to meet the desired requirement and standards of the Fire Code. E. DISCUSSION OF ISSUES 1. Building Classification and Construction The ZPI PHILIPPINES, INC. building facility is classified as a type IV construction wherein it stated and defined that buildings shall be of steel, iron, concrete or masonry construction. Walls, ceilings, and permanent partitions shall be of incombustible fire resistive construction. 2. Hazards of Content & Combustible Loadings There were least contents of hazard loadings, because the raw materials and packaging materials that are used by the company is not highly toxic and chemical hazard to occupational safety. Their EMS (Environmental Management System) is aligned to the program that is standard to the required and desired compliances. In such cases, data’s of MSDS (Material Safety Data Sheet) is secured as customer requirement in any of the companies purchases. Mr. Edgar T. Orellano Factory Maintenance

Occupancy Classifications ZPI owned the building, and it is classified as Industrial type of firm where it is defined as used for manufacturing of all kinds of products devoted to operations such as processing, assembling, mixing, packaging, finishing and repairing/reworking. Wherein ZPI is devoted to manufacturing of plastic injected parts from proces to finished products. It is sub-classified as Special Purpose Industrial Occupancy, where devoted to a particular operation, except high hazard. Its capacity according to Places of Assembly: Occupancy is classified as Class C, wherein the company has 90 workers (as of August 2010) and/or employees that covers under/ that falls under the Classification of Class C Occupancy (Class C: 50-300 employees). 4. Separation of Building Spaces ZPI is separated by the perimeter fence from its neighboring company’s, and its partition made from cyclone wire and concrete wall fencing. It is separated from the istration office located at the 2nd floor of the 1st building, and separated from the production area. 5. Exit & Egress a. Number of Active exits ZPI has a total of 17 Active and unobstructed exits. This exit also has a lay-out throughout the whole company for easy access of the employees in case of fire emergencies. b. Maintenance records done on egress component such as doors and enclosed stairways. Monthly checking and inspection by the maintenance section to the egress of the company to secure availability of the said pathways. This is done and included in the checking of fire extinguishers. A record of the following check-up is secured by the maintenance section staff. 6. Fire Suppression The testing records are available to secure that all fire suppression systems are well equipped and is ready for usage. 7. Fire detection, alarm communication & evaluation system The said system is done when there is a drill and training set by PEZA Emergency Response Unit and ZPI Company. There were still cases for improvement in the monitoring, checking, and inspection of the following components that will be done to ensure the standard set and required in the Fire Code and the National Building Code. 8. Emergency power supply In case of power failure, the company has provided a generator set (standby) that will be used and is checked and tested as need as arises by the authorized personnel.
